Please note that Building/Inspections is now located at the Municipal Services Center, 3635 - 4th Avenue.

 

(Please use the same bond form and contractor registration for each trade)

 

GENERAL CONTRACTOR

$5000 License/Permit Bond
MUST be on City of Moline bond form and have insurance seal, signatures, and power of attorney form attached.

Continuous bond until cancelled by 60-day written notice.

Certificate of Insurance
Show City of Moline as certificate holder with 30-day written cancellation notice:

Public liability and property damage insurance
$100,000 per occurrence of property damage.
$300,000 per occurrence for injury to persons.

Worker’s compensation insurance
Certificate from insurance provider

OR Certificate of Approval as a self-insurer issued by Illinois Industrial Commission.

OR if applicant has no employees, submit sworn statement stating that fact.

$75.00 Contractor Registration
Annual fee payable to “City of Moline”

Roofing License
Copy showing current expiration date issued by the State of Illinois
(required for any roofing job, except sheet metal roofs.)

WRECKING CONTRACTOR

$5000 License/Permit Bond
Must be on City of Moline bond form and have insurance seal, signatures, and power of attorney form attached.

Continuous bond until cancelled by 60-day written notice.

Certificate of Insurance
Show City of Moline as certificate holder with 30-day written cancellation notice.

$50,000 per occurrence for damage to property
$300,000 per person for injury to persons
$500,000 per occurrence for injury to persons

$75.00 Contractor Registration
Annual fee payable to “City of Moline”

MOVING CONTRACTOR

$25,000 License/Permit Bond
MUST be on City of Moline bond form and have insurance seal, signatures, and power of attorney form attached.

Continuous bond until cancelled by 60-day written notice.

“Hold Harmless” agreement

Certificate of Insurance
Show City of Moline as certificate holder with 30-day written cancellation notice:

Public liability and property damage insurance
$300,000 per occurrence of property damage.
$500,000 per occurrence for injury to persons.

• Certified check to Finance Dept
Amount determined by Finance Officer to offset costs expended by City personnel in assisting with moving operation (Chapter 8, Section 5702, Moline Code of Ordinances).

• Trees that need to be trimmed or removed
Said building moving shall first be approved by the Superintendent of Parks and the work performed in connection therewith shall be performed by reliable, bonded and insured tree trimmers, approved by the Superintendent of Parks and as by Moline Code of Ordinances.


PLUMBING CONTRACTOR

• $50.00 annual administrative filing fee.

• Submit copy of State of Illinois plumbing license showing current expiration date for
self (058) & business (055).

MECHANICAL CONTRACTOR

• License shall be issued ONLY to an individual, not to a corporation or firm.

• $5000 License/Permit Bond
Must be on City of Moline bond form and have insurance seal, signatures, and power of attorney form attached.

Continuous bond until cancelled by 60-day written notice.


• Certificate of Insurance
Show City of Moline as certificate holder with 30-day written cancellation notice:

Public liability and property damage insurance
$100,000 for each occurrence of property damage
$300,000 for each occurrence for injury to persons

Submit an affidavit verifying 5-year minimum as mechanical contractor.

• City of Moline Mechanical License
$50.00 annual fee for each mechanical license
(forced air, hydronics, fireplace installation).
 

Provide a satisfactory affidavit that the applicant has passed the master's mechanical examination through Thomson Prometric or the International Code Council National Contractor Trades Examination Program, Standard Master Mechanical exam.


Submit an affidavit verifying 5-year minimum as mechanical contractor.


• To conduct business in Moline without Moline Mechanical License
$50.00 annual administrative filing fee.

Submit copy of the mechanical license from another city within the State of Illinois showing current expiration date.

Submit an affidavit verifying 5-year minimum as mechanical contractor.

ELECTRICAL CONTRACTOR
 

License shall be issued ONLY to an individual, not to a corporation or firm.

• $5000 License/Permit Bond
Must be on City of Moline bond form and have insurance seal, signatures, and power of attorney form attached.

Continuous bond until cancelled by 60-day written notice.

Show City of Moline as certificate holder with 30-day written cancellation notice:

Public liability and property damage insurance
$100,000 per occurrence of property damage.
$300,000 per occurrence of personal injury or bodily harm.

Worker’s compensation insurance

OR Certificate of Approval as a self-insurer issued by Illinois Industrial Commission

OR if applicant has no employees, submit sworn statement stating that fact.

City of Moline Electrical License.
$50.00 annual filing fee.

Provide a satisfactory affidavit that the applicant has passed the master's electrical examination through Thomson Prometric or the International Code Council National Contractor Trades Examination Program, Standard Master Electrical exam.


Submit an affidavit verifying a minimum of 5 years experience as an electrical contractor.


To conduct business in Moline without Moline Electrical License.
$50.00 annual administrative filing fee.

Submit a copy of the electrical license from another city within the State of Illinois showing current expiration date.

Submit an affidavit verifying a minimum of 5 years experience as an electrical contractor.


Proof of Master Block Test is not required.
